The Plan

 

A complete wellness examination where you will be screened for over 60 conditions. At the completion of the Plan process, you will know what health issues need to be treated immediately and what issues need to be re-evaluated in the future. You will also receive educational materials about general health and any issues you are diagnosed with. We will also lay out a timeline for future recommended tests.

 

The wellness plan is broken down into 2 visits. The first includes the HRA questionnaire review, and lab work. At the second visit, lab results will be given along with counseling about current conditions and prevention.   Read more

 

 

These examinations are by appointment only. An initial appointment must be scheduled prior to taking the Questioniare. We will bill your wellness benefit, if your insurance has one.
